A 7-year-old boy presented with a rare form of alopecia areata (AA) known as linear alopecia areata (LAA), characterized by a linear pattern of hair loss on the scalp. The diagnosis was confirmed through dermatoscopy and histopathology, which revealed a lymphocytic infiltrate and a "swarm of bees" pattern without follicle destruction. Differential diagnoses considered included trichotillomania, congenital triangular alopecia, linear morphea, and linear cutaneous lupus erythematosus. The patient was treated with a combination of 5% minoxidil, 0.015% retinoic acid, and 0.05% clobetasol, resulting in full hair regrowth within 2 months and complete recovery after 1 year. The case highlights the importance of accurate diagnosis and treatment in managing LAA, especially in pediatric patients.
