TLDR Use topical treatments for mild acne and oral treatments for moderate to severe acne, avoiding antibiotic monotherapy.
The 2024 American Academy of Dermatology guidelines offer a framework for managing acne vulgaris in Canada, emphasizing evidence-based practices. For mild acne, topical treatments like benzoyl peroxide and antibiotics are strongly recommended, while retinoids and other agents have conditional support. Oral treatments such as doxycycline and isotretinoin are strongly recommended for moderate to severe cases, with isotretinoin being the standard for severe acne and scarring. The guidelines discourage antibiotic monotherapy, suggesting combination with benzoyl peroxide to reduce resistance. Special considerations are advised for pregnant individuals, people with skin of color, and those undergoing gender-affirming therapy.
