Acute Localised Exanthematous Pustulosis (ALEP) was described as a less recognized variant of Acute Generalised Exanthematous Pustulosis (AGEP), often triggered by drugs like amoxicillin-clavulanic acid. A case study of a 19-year-old boy demonstrated ALEP's presentation as non-follicular pustules on an erythematous base, resolving spontaneously within 7-10 days after discontinuing the drug. The study highlighted dermoscopic features, such as milky-white globules and peripheral dotted vessels, aiding in diagnosis. ALEP was linked to a Type IV hypersensitivity reaction, involving CD8+ T cells and neutrophils, with a potential genetic predisposition shared with pustular psoriasis.
