TLDR Lichen spinulosus, usually seen in young people, can rarely occur in adults, as shown by a 52-year-old woman with a rough, bumpy rash.
Lichen spinulosus (LS) was an uncommon skin condition primarily affecting children and adolescents, but it was rare in adults. It manifested as hypopigmented or skin-colored follicular papules with a sandpaper-like texture. The condition involved a lymphohistiocytic infiltrate in the dermis around hair follicles. A rare case was reported in a 52-year-old woman who exhibited a rough, bumpy, itchy rash on her trunk and extremities, characterized by clusters of hyperkeratotic follicular-based spiny papules. Histological examination revealed dilated hair follicles with keratotic plugs and a dense perifollicular lymphohistiocytic infiltrate extending into the follicular epithelium.
