The review examines the effects of 5α-reductase inhibitors, finasteride and dutasteride, on semen parameters and their reversibility. Finasteride, particularly at 1 mg, causes mild and reversible reductions in semen volume, sperm concentration, and total sperm number, with minimal impact on sperm motility and morphology. Higher doses (5 mg) may cause transient motility reductions. Dutasteride has more significant effects, including greater declines in semen volume, motility, and total sperm number, with incomplete recovery after long-term use (18-20 months). Gonadotrophin levels remain stable, indicating preserved hypothalamic-pituitary function. The findings suggest caution with prolonged dutasteride use in men planning conception.
