The document reported a case of chronic pulmonary histoplasmosis in Portugal involving a patient with a history of various health conditions, including type 2 diabetes, hypertension, and past infections like typhoid fever and malaria. The patient, who had regular contact with chickens and past exposure to pigeons and bats in Guinea-Bissau, experienced fatigue and dyspnea over 2 years. Despite several examinations, a conclusive diagnosis was initially elusive. The patient presented with symptoms such as 90% O2 saturation, emaciation, and disperse crepitations in the lungs, alongside a holosystolic heart murmur.
