Clinical Predictors of Outcome in Nonsegmental Vitiligo: A Prospective Cohort Study

    TLDR Certain skin and family history markers predict vitiligo severity and treatment needs.
    In a prospective cohort study of 275 patients with non-segmental vitiligo (NSV) followed for 12 months, researchers identified clinical markers that predict disease progression and impact. Key findings include that leukotrichia, trichrome and confetti lesions, koebnerization, and involvement of mucosal, acral, and periorificial areas are associated with greater disease extent, poor repigmentation, higher treatment dependency, and increased noticeability. Leukotrichia was a consistent predictor of poor repigmentation and high cosmetic concern. A family history of autoimmune disease predicted disease progression and treatment dependency, while early-onset vitiligo was linked to less skin involvement but greater psychosocial burden. These markers can help stratify patients into prognostic subgroups, aiding in individualized treatment and counseling.
