TLDR Clomiphene testing can help find ovarian causes of high testosterone in postmenopausal women.
A 62-year-old postmenopausal woman with severe hyperandrogenism and high testosterone levels underwent a clomiphene test to determine the origin of her condition. Despite normal serum DHEA-sulfate levels and no ovarian lesions on imaging, her testosterone levels increased significantly after clomiphene administration, indicating an ovarian source. Subsequent bilateral oophorectomy confirmed a Leydig cell tumor. This case suggests that clomiphene testing can help identify ovarian origins of hyperandrogenism in women with low gonadotropin levels.
