Dermoscopic and Histopathological Correlation in Cicatricial Alopecias: Unveiling Diagnostic and Therapeutic Insights

TLDR Trichoscopy effectively diagnoses and monitors cicatricial alopecias, reducing the need for biopsies.
This study highlights the effectiveness of trichoscopy, a non-invasive dermoscopic imaging technique, in diagnosing cicatricial alopecias. Conducted from September to November 2023 with 16 patients, the study found that trichoscopy accurately identified unique features of cicatricial alopecia, such as loss of follicular openings and follicular pustules, which correlated with histopathological findings. The technique proved superior in early diagnosis, monitoring disease progression, and assessing therapy response, reducing the need for unnecessary biopsies. The study included cases of traumatic alopecia, lichen planopilaris, discoid lupus erythematosus, and acne keloidalis nuchae, emphasizing trichoscopy's role in improving patient outcomes by facilitating timely therapeutic interventions.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

A user in France is frustrated because three dermatologists refused to prescribe finasteride for their hair loss, suggesting it's not androgenetic alopecia (AGA) and offering expensive vitamin treatments instead. Other users suggest buying hair loss treatments like minoxidil and finasteride from various online pharmacies and considering a gender-affirming care physician for more understanding treatment options.
