TLDR Diagnose PCOS in teens using irregular periods and high androgen levels, not ultrasound.
The literature review examined the challenges of diagnosing Polycystic Ovary Syndrome (PCOS) in adolescents due to overlapping symptoms with normal puberty. It criticized the use of the Rotterdam criteria, particularly pelvic ultrasound, as inappropriate for adolescents and suggested using the NIH criteria, focusing on irregular menstrual cycles and hyperandrogenism. Anti-Müllerian Hormone (AMH) levels were considered a potential diagnostic aid if main criteria were met. The review highlighted the importance of considering ethnic and phenotypic differences and recommended waiting two years post-menarche to diagnose oligomenorrhea. It emphasized the need for standardized guidelines and further research to improve diagnosis and management in adolescents, noting the potential metabolic risks associated with menstrual irregularities.
