Diffuse Unpatterned Alopecia: Practical Diagnostic Considerations

    Diffuse unpatterned alopecia (DUPA) is characterized by global follicular miniaturization and loss of donor dominance, making hair transplantation unsafe. This review suggests a comprehensive diagnostic approach using clinical mapping, trichoscopy, trichometry, AI-assisted counts, and histopathology to assess donor-area safety. Key indicators distinguishing DUPA from other conditions include hair shaft diameter diversity (HSDD) ≥20% and an occipital terminal:vellus ratio <4:1. A pragmatic workflow involving photography, trichoscopy, and biopsies is recommended to avoid misclassification and inappropriate transplantation. Management should focus on medical treatment and reassessment, with surgical options contraindicated. Future directions include non-invasive techniques and AI for improved diagnosis and monitoring, with an emphasis on standardizing diagnostic criteria and risk assessment.
