When JAK Inhibitors Are Off The Table: Durable Remission Of Severe Alopecia Areata With Dexamethasone Oral Mini-Pulse And A Focused Review

Dexamethasone oral mini-pulse (OMP) therapy is an effective alternative for treating severe alopecia areata (AA) in patients who cannot use JAK inhibitors. A case study of a 52-year-old woman with systemic lupus erythematosus showed significant improvement in her SALT score after four months of OMP therapy, with sustained hair regrowth at 12 months without maintenance. A review of 14 clinical studies found that 89% of patients improved with a 71% median SALT reduction by week 24, although relapse occurred in about 60% of responders post-treatment. OMP is a safe, cost-effective outpatient option, with mild and manageable safety profiles, emphasizing the need for patient selection, early intervention, and relapse planning. Further research is needed to optimize treatment protocols and assess efficacy in immunosuppressed populations.
