TLDR Tretinoin may help some who don't respond to minoxidil alone, but it's not a first choice due to possible irritation.
This review examines the use of topical tretinoin in combination with topical minoxidil for treating androgenetic alopecia (AGA). While pharmacokinetic evidence suggests that tretinoin increases minoxidil absorption, clinical data on the efficacy of this combination is limited. One randomized controlled trial found no significant difference between dual therapy and minoxidil alone, though smaller studies suggest some nonresponders to minoxidil may benefit from the addition of tretinoin. Social media analysis indicates positive perceptions of the combination, but concerns about tolerability are common. Overall, while not recommended as a first-line treatment, tretinoin may be considered for patients who do not respond to minoxidil alone, with caution regarding potential irritation.
