TLDR Stopping afatinib improved a rare scalp condition in a patient.
This document details a rare case of erosive pustular dermatosis of the scalp (EPDS) in a 57-year-old woman undergoing afatinib therapy for stage IVA lung cancer. The patient developed thick, yellowish crusts on the scalp, which were confirmed as EPDS. Despite treatment with corticosteroids and antibiotics, significant improvement was only achieved after reducing and discontinuing afatinib. This case highlights the need for early recognition and management of EPDS in patients on afatinib to prevent scarring alopecia. The report underscores the importance of monitoring for rare adverse effects of afatinib, particularly on the scalp, as conventional treatments were ineffective, and resolution was only observed after stopping the medication.
