Erosive Pustular Dermatosis of the Scalp: A Case Report

    Erosive pustular dermatosis of the scalp (EPDS) is a rare inflammatory condition characterized by sterile pustules, erosions, and crusts, leading to scarring alopecia, primarily affecting older women. This case report discusses a 55-year-old female patient with a large atrophic erythematous plaque on the scalp, covered with pustules and crusts. The condition developed over three months without pain but with moderate itching. Tests for various infections and autoimmune conditions were negative, but Staphylococcus aureus was found in bacterial cultures. Histopathology showed a polymorphic inflammatory infiltrate. Treatment with oral antibiotics and topical corticosteroids resulted in significant improvement, though scarring alopecia remained. EPDS is diagnosed by exclusion, requiring the elimination of other potential conditions, and prompt diagnosis and treatment can reduce scarring.
