A 30-year-old patient with extensive alopecia areata, characterized by a large alopecia plaque on the scalp, was effectively treated with a combination of methotrexate, dexamethasone, and minoxidil. The treatment plan included methotrexate 15 mg/week, folic acid 5 mg daily except on the day of methotrexate, dexamethasone 5 mg twice a week, and daily application of 5% minoxidil hair lotion. After 48 weeks of treatment, the patient showed satisfactory improvement, highlighting the effectiveness of this therapeutic approach for severe forms of alopecia areata.
