TLDR Follicular vitiligo causes hair to gray without skin color loss.
This case report discusses a rare variant of vitiligo known as follicular vitiligo, which involves the selective destruction of melanocytes within hair follicles, leading to leukotrichia (white or gray hair) without initial skin depigmentation. The report focuses on a 40-year-old Indian male who presented with asymptomatic graying of hair and depigmented patches on the trunk, lower limbs, and genital mucosa. Diagnosis was achieved through clinical examination and dermoscopy, highlighting the diagnostic challenges due to its resemblance to conditions like alopecia areata. The report underscores the importance of recognizing the morphological diversity of vitiligo and its rare variants.
