This study investigated the effects of finasteride 1 mg/day on male fertility, involving 465 men aged 18–60 years, with 69 finasteride users and 396 controls. Results showed that while finasteride users had similar metabolic and hormonal profiles to controls, they experienced significantly reduced semen quality, including lower sperm concentration, total sperm count, total motile sperm count, and altered sperm morphology. Despite these impacts on sperm production, finasteride did not affect endocrine, sexual, or psychological functions.
