This retrospective review of 28 patients with Infantile Perianal Pyramidal Protrusion (IPPP) at Mayo Clinic highlights the condition's rarity and potential for misdiagnosis. IPPP primarily affects prepubertal females and can be mistaken for other conditions, leading to unnecessary aggressive treatments. The study found that 21% of cases were congenital, 75% were acquired, and 4% were associated with lichen sclerosus. A significant portion of acquired cases (71%) had a history of constipation. Most patients were managed conservatively, with only 14% receiving topical treatments. The study emphasizes the importance of recognizing IPPP to avoid undue stress and trauma from misdiagnosis and inappropriate interventions.
