TLDR Actinic prurigo cheilitis can occur in elderly Asians and can be treated with hydrocortisone cream and sun protection.
This case study reports an unusual presentation of actinic prurigo (AP) in a 67-year-old Thai female, characterized by isolated cheilitis with chronic, painful ulcers on the lower lip, a presentation not previously documented in elderly Asian patients. Despite normal phototesting results, histopathological examination confirmed AP cheilitis. The patient achieved complete remission after two months of treatment with topical 1.25% hydrocortisone cream and strict sun protection. This case emphasizes the need to consider AP in the differential diagnosis of chronic lip lesions, even in atypical demographics, and highlights the effectiveness of conservative management.
