The study evaluates the novel "single anastomosis sleeve ileal" (SASI) bypass surgery, performed on 25 patients (17 women, 8 men) with a mean age of 40.1 years and a mean preoperative BMI of 43.7 kg/m2. Results showed a significant decrease in BMI by 6.1 kg/m2 at 3 months and 11.6 kg/m2 at 6 months post-surgery. Comorbidities such as hypertension and prediabetes showed improvement, with hypertension resolving in 9 out of 11 cases and prediabetes normalizing in 2 out of 2 cases. Postoperative complications included reflux, sleep disorder, fatigue, and hair loss, but no lasting diarrhea was observed. The SASI bypass effectively combines restrictive and malabsorptive techniques, offering significant weight loss and comorbidity resolution with minimal risk of impaired absorption, suggesting it could become a leading bariatric procedure.
