This case report discusses a 42-year-old Yemeni man with hepatic steatosis who experienced liver injury potentially linked to khat use. Khat, commonly used for its stimulant effects, contains harmful compounds that can cause liver toxicity. The patient, who chewed khat more frequently during a recent trip, presented with elevated liver enzymes and diffuse hepatic steatosis. Despite no personal or family history of liver disease, his condition improved with treatment. The report emphasizes the need for awareness of khat's potential to exacerbate liver conditions, especially in the context of obesity and fatty liver disease, and suggests that chronic khat use may accelerate progression to cirrhosis.
