TLDR Hair extensions can worsen scalp conditions and should be avoided in those with alopecia.
This case report discusses a 29-year-old female with alopecia areata who developed lichen planopilaris after using glued hair extensions. Initially treated with baricitinib, she experienced hair regrowth, but reduced her medication dose, leading to hair shedding. To conceal this, she used hair extensions, which resulted in a new alopecic patch diagnosed as lichen planopilaris. Treatment included discontinuing hair extensions, resuming baricitinib, and using prednisolone and topical treatments, leading to stabilization and hair regrowth. The report highlights the potential risk of hair extensions in patients with underlying scalp conditions and the importance of treating the primary cause of alopecia.
