TLDR Lichen Planus in siblings may be influenced by genetics and environment.
This study reports on three Iranian siblings with different clinical forms of Lichen Planus (LP), suggesting a potential genetic and environmental influence on the disease. The siblings included a male with Lichen Planus Pigmentosus (LPPigm), a female with LPPigm and Lichen Planopilaris (LPP), and another female with Frontal Fibrosing Alopecia (FFA). Diagnoses were confirmed through histopathological examination, and treatment involved sunscreen, topical corticosteroids, pimecrolimus, minoxidil, and finasteride. This unique case series highlights the need for further research into the genetic and environmental factors of LP.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
