This case report describes the MRI-documented re-continuity of a complete ACL rupture in a 65-year-old male following delayed platelet-rich plasma (PRP) treatment. The patient received five image-guided PRP treatments starting six weeks post-injury. By 14 weeks, MRI showed improved fiber organization and apparent re-continuity of the ACL, with further maturation observed at 50 weeks. The patient regained normal function by 14 weeks. While this case does not establish causation or recommend non-operative management for all ACL ruptures, it highlights the potential for ACL healing in older patients with delayed PRP treatment, warranting further research into treatment protocols and outcomes.
