Management of Folliculitis Decalvans: The EADV Task Force on Hair Diseases Position Statement

    TLDR Control inflammation and prevent further hair loss in folliculitis decalvans using medications and therapies.
    Folliculitis decalvans is a chronic and relapsing form of scarring alopecia affecting 2.8% of hair loss patients, where the primary goal of treatment is to control inflammation and prevent further hair loss, as regrowth is not expected. The EADV Task Force on Hair Diseases provides recommendations for managing this condition, suggesting oral antibiotics for moderate to severe inflammation and oral glucocorticosteroids for highly active disease. Oral isotretinoin is recommended for mild active disease and refractory cases. Additional treatments include photodynamic therapy, biologics, JAK inhibitors, oral dapsone, hydroxychloroquine, or cyclosporine, with topical or intralesional corticosteroids as adjuncts. Surgical excision, laser therapy, and hair transplantation are options for patients with inactive disease.
