TLDR Microneedling with Triamcinolone acetonide is an effective, less invasive, and safe alternative to direct injections for treating alopecia areata.
The study compared the effectiveness of intralesional triamcinolone acetonide versus microneedling with triamcinolone acetonide for alopecia areata treatment in 50 patients. The results showed a statistically significant reduction of the SALT score and all dystrophic hair markers of alopecia areata by dermoscopy at the end of the study versus baseline in both groups. However, the difference between the microneedling group and the intralesional group concerning clinical response was statistically insignificant. The study concluded that microneedling, especially when combined with Triamcinolone acetonide, is an effective treatment for alopecia areata, providing a comparatively inexpensive, safe, less painful, less invasive tool with no serious complications.

Microneedling is more effective when combined with minoxidil, especially for temple regrowth, but is considered near-useless on its own. Users suggest combining microneedling with oral minoxidil and dutasteride for better results, while some caution against potential scalp damage.

Avoid sun exposure for 7-14 days after microneedling the scalp, using a hat for protection. Sunscreen on the scalp can be messy and irritating.
