    The study identifies a unique circulating microRNA (miRNA) signature that distinguishes severe alopecia areata (AA) from other inflammatory skin diseases, offering potential non-invasive biomarkers for diagnosis and stratification. Researchers found 19 miRNAs significantly downregulated in severe AA, with 10 miRNAs validated for high specificity. A machine learning model using the top four miRNAs achieved a high classification accuracy (AUC = 0.94). The study also highlights dysregulated immune, metabolic, and signal transduction pathways, suggesting potential therapeutic targets such as kinase inhibitors and antioxidants. This research provides a foundation for developing non-invasive diagnostic tools and future treatments for severe AA.
