The study developed a new 7-point grading scale for evaluating overall hair density, involving 1661 subjects, to address limitations of existing scales in detecting early-stage hair loss and treatment response. Validated for reliability and sensitivity, the scale ranges from Grade 1 (extremely sparse hair) to Grade 7 (extremely dense hair) and is applicable to both genders, focusing on the frontal and vertex regions. It showed excellent intra-rater and inter-rater reliability, with Kappa coefficients between 0.971 and 0.990, and a Fleiss-Kappa coefficient of 0.838. In a clinical trial with 120 subjects, the scale demonstrated significant sensitivity to treatment response. Although currently applicable to Chinese individuals with straight hair, the scale offers a precise tool for assessing alopecia extent and treatment efficacy, with future research aimed at broader validation.
