TLDR Using dutasteride during hair transplants improves results and reduces hair loss.
This article reviews the medical literature on the perioperative use of dutasteride, both orally and intradermally (mesotherapy/microneedling), to prevent shock loss and enhance aesthetic outcomes in hair transplant patients with androgenetic alopecia (AGA). The review included clinical trials, systematic reviews, cohort studies, and case reports from 2018 to 2025. Findings indicate that dutasteride, a dual inhibitor of type I and II 5-alpha-reductase isoenzymes, is more effective than finasteride in suppressing dihydrotestosterone (DHT), aiding in follicular stabilization before and after surgery. Mesotherapy with dutasteride showed comparable efficacy to oral administration with fewer systemic side effects. Combining both methods yielded better results than using either alone. The perioperative dutasteride protocol is promising for reducing shock loss and improving graft survival, though specific randomized clinical trials are still limited.
