Case Report: Pulsed Electric Field Treatment of Metastatic Liver Disease Demonstrating a Possible Abscopal Response

    This case report describes a 49-year-old female with metastatic adrenocortical carcinoma in the liver who experienced an abscopal effect following pulsed electric field (PEF) ablation. Despite previous treatments, her disease progressed, leading to the use of pembrolizumab and mitotane. After PEF treatment, imaging showed complete resolution of three out of four untreated lesions, suggesting a potential immunomodulatory effect of PEF. This indicates that PEF could serve as an adjunct to systemic immunotherapy in treating liver tumors. The report emphasizes the need for further studies to understand the mechanisms and benefits of PEF in cancer treatment, noting limitations such as a single patient, short follow-up, and lack of immune response monitoring.
