    Pilomatricoma is a rare benign skin tumor originating from hair follicle matrix cells, often posing a diagnostic challenge due to its varied clinical presentations. It is more common in females and typically appears as a painless, firm, and slowly growing lesion. Histopathological analysis reveals basaloid cells at the periphery and shadow cells centrally, while immunohistochemical studies focus on cytokeratin expression related to hair matrix differentiation. The primary treatment is complete surgical excision, which generally leads to favorable outcomes and reduces recurrence risk. This case study highlights a 32-year-old male with Pilomatricoma in the lower left orbital region, experiencing swelling for 7 months, emphasizing the importance of clinician awareness for timely diagnosis and treatment.
