This document presents two case reports of patients with refractory dissecting cellulitis of the scalp (DCS) successfully treated with risankizumab, an IL-23 inhibitor. DCS is a rare, chronic inflammatory condition that can lead to scarring alopecia and significantly impact quality of life. Traditional treatments often result in frequent relapses. In these cases, a 26-year-old male and a 17-year-old boy, both of whom had not responded to previous therapies, showed marked clinical improvement and hair regrowth after receiving risankizumab injections. The Dermatology Life Quality Index scores improved significantly for both patients, indicating enhanced quality of life. This suggests that risankizumab may be an effective treatment option for recalcitrant DCS.
