Secondary Cicatricial Scalp Alopecia Caused by Brunsting-Perry Pemphigoid: A Case Report

Brunsting-Perry pemphigoid, a rare bullous dermatosis affecting the head, scalp, and neck, can lead to cicatricial alopecia, as demonstrated in a case involving a 68-year-old Caucasian woman with a 10-year history of patchy hair loss. Diagnostic tools such as dermoscopy and skin biopsy revealed characteristic signs, including a "fried-egg sign" and subepidermal blistering with dermal inflammation. Immunopathology indicated strong deposits of C3c and IgM. Treatment with intralesional betamethasone and maintenance with topical mometasone furoate lotion proved effective. This case highlights the potential underdiagnosis of Brunsting-Perry pemphigoid as a cause of scarring alopecia due to limited literature on the condition.
