Mechanical Stimulation of the Scalp in Androgenetic Alopecia: A Narrative Review of Massage, Tension, and Mechanotransduction

    This narrative review examines the evidence for mechanical stimulation of the scalp in treating androgenetic alopecia (AGA), focusing on manual scalp massage, tension, and mechanotransduction. The review highlights that while the dermal papilla is responsive to mechanical stimuli, the clinical evidence for manual massage is limited, with only one small uncontrolled study involving nine men and a retrospective survey showing 68.9% reporting stabilization or regrowth. The galea-tension hypothesis and circulation arguments are discussed but lack strong causal evidence. Microneedling, which operates through controlled wound healing, has more robust trial support compared to massage. Motorized devices are theoretically plausible but lack specific efficacy trials. Overall, mechanical stimulation is considered a low-risk adjunct therapy with a credible mechanism but requires more rigorous, controlled trials to establish its efficacy.
