This document reports a rare presentation of alopecia areata (AA) where multiple hair shafts exhibit segmental heterochromia and changes in calibre. These pigmentary and calibre alterations indicate an intermittent low-level inflammatory process in diffuse AA, with phases of recovery during which the hair fibre returns to its normal state.
