In Vitro Stability and Preclinical Safety Evaluation of High-Dose Intravitreal Topotecan in Rabbits: Impact of Dose and Concentration

    TLDR Lower doses of topotecan are safe for eyes, but higher doses may harm the retina.
    This study assessed the stability and safety of high-dose intravitreal topotecan in 24 New Zealand White rabbits, with a focus on its application for treating retinal tumors. Topotecan solutions (1-4 mg/ml) were stable for 1 month at 4°C and –20°C. Rabbits received three monthly injections of 50 μg, 100 μg, or 200 μg. Doses up to 100 μg were safe, showing no systemic toxicity or ocular damage, while 200 μg doses caused retinal alterations and decreased ERG wave amplitudes. The results suggest that doses up to 100 μg are safe for use, but 200 μg doses may lead to retinal changes, indicating the need for careful dose management in clinical settings.
