A 6-year-old child presented with a trichilemmal cyst on the right upper eyelid, a rare occurrence in the pediatric age group. The cyst was a painless, hard, oval-shaped swelling that was adherent to the skin but mobile from the underlying tissue. Histopathological examination confirmed the diagnosis, revealing a cyst wall lined by stratified squamous epithelium with dense laminated keratin and dystrophic calcification. Trichilemmal cysts, typically found in hairy regions like the scalp and face, are uncommon in the eyelid, especially in children, and can be misdiagnosed as chalazion. This case highlights the importance of considering trichilemmal cysts in atypical eyelid lesions in children.
