This case series reports on 10 female patients with non-marginal traction alopecia, characterized by asymptomatic focal hair loss in the middle of the scalp, with otherwise normal long hair. Despite denying common causes like tight hairstyles or chemical treatments, trichoscopic and histopathological evaluations confirmed traction alopecia. The study suggests that the weight of very long hair, termed "Rapunzel alopecia," may contribute to this condition. Patients were advised to cut their hair shorter and use a topical 2% minoxidil solution, which improved their condition within months. This highlights the importance of recognizing non-marginal traction alopecia as a distinct form of hair loss due to prolonged physical pressure on the scalp.
