This case report describes a rare occurrence of a 72-year-old postmenopausal woman with a steroid cell tumor of the ovary, leading to virilization symptoms such as hirsutism and male-pattern alopecia. The tumor was hormonally active, with elevated serum testosterone levels, and was confirmed through imaging, biochemical evaluation, and histopathology. Surgical intervention resulted in normalized testosterone levels and regression of virilizing features. The report underscores the importance of a multidisciplinary approach, including integrated imaging, biochemical profiling, and pathological correlation, for diagnosing and managing androgen-secreting ovarian tumors in postmenopausal women, and recommends long-term follow-up to monitor for recurrence.
