Ovarian steroid cell tumors were rare and life-threatening, comprising about 0.1% of all primary ovarian tumors, and typically presented in premenopausal women with virilization. This case report described a 58-year-old postmenopausal woman who exhibited virilization symptoms and had elevated serum testosterone levels. Ultrasound revealed 2 follicles in the left ovary, and she underwent bilateral salpingo-oophorectomy and hysterectomy. Histology confirmed a benign steroid cell tumor. The occurrence of new-onset, rapidly progressing hyperandrogenism was rare in postmenopausal women, and diagnosis required consideration of adrenal and ovarian malignancies. Surgical treatment was effective for stromal luteoma, a rare benign ovarian tumor.
