Testosterone therapy in transmen often leads to moderate to severe acne, necessitating oral treatments, while feminizing hormones in transwomen can reduce severe acne but may cause skin dryness and irritation, requiring topical treatments. A group of five experts reviewed 68 publications to address acne management in transgender patients, emphasizing a multidisciplinary approach involving various specialists. The study highlights that "transgender" acne treatment differs from "classic" acne treatment, with a high risk of relapse after stopping treatment, justifying maintenance with topical treatments and mandatory use of ceramide-enriched emollients. Despite challenges, effective treatment options are available.
