This case series highlights severe cutaneous adverse reactions in four adult women following the intradermal injection of unregulated exosome-based formulations, which are popular in aesthetic dermatology for their regenerative properties. The patients developed persistent erythema, nodules, granulomatous inflammation, and scarring, with treatments including corticosteroids, laser therapy, and surgical removal. Despite these interventions, all patients experienced incomplete resolution and residual scarring. The study emphasizes the risks associated with the off-label use of these products and calls for regulatory enforcement, practitioner education, and increased public awareness to ensure patient safety.
