TLDR Exclamation mark hairs and broken hairs best indicate active alopecia areata.
This study on 137 patients with alopecia areata (AA) identifies trichoscopic markers associated with disease activity, highlighting exclamation mark hairs as the most reliable indicator of active AA, with a sensitivity of 91.9% and specificity of 90.7%. Broken hairs also significantly indicate activity, while other markers like yellow dots and short vellus hairs are more common in stable AA. The study underscores trichoscopy's potential as a reliable tool for assessing AA activity and suggests the need for a larger sample size and an objective gold standard for improved validity.
