TLDR Abrocitinib may effectively treat stubborn alopecia universalis.
This case report describes the successful treatment of alopecia universalis (AU) in a 30-year-old woman with a history of drug reaction with eosinophilia and systemic symptoms (DRESS) using abrocitinib, a selective JAK1 inhibitor. The patient, who also had bipolar disorder and hepatic issues, initially experienced full-body hair loss following DRESS. Previous treatments, including minoxidil and tofacitinib, were ineffective. However, after 6 months of abrocitinib therapy, significant hair regrowth was observed without adverse effects. This case highlights abrocitinib's potential as a promising treatment for AU, especially in cases resistant to other therapies, and suggests a possible link between eosinophils and DRESS-induced alopecia.
