This report discusses a rare case of a giant atypical proliferating pilomatrixoma on the eyelid of a 47-year-old male. The mass, measuring 7×10 cm, was surgically removed and identified as an atypical proliferating pilomatrixoma with minimal infiltrating margins and focal cytological atypia. No recurrence was observed one year after surgery. Pilomatrixomas, which originate from hair follicles, are rare and can resemble other periocular lesions, with a risk of malignant transformation. Therefore, surgical excision and histopathological evaluation are essential for accurate diagnosis and management, and long-term follow-ups are advised to monitor for recurrence or malignant progression.
