Chronic Erosions and Alopecia: A Case of Erosive Pustular Dermatosis of the Scalp in an Elderly Male

    Erosive pustular dermatosis of the scalp (EPDS) is a rare condition affecting mainly the elderly, characterized by pustular, crusted lesions leading to scarring alopecia. This case study reports a 65-year-old male with EPDS, presenting with scalp erosions and pustules following minor trauma. Diagnosis was confirmed through clinical examination and histopathology, revealing neutrophil-rich inflammation and fibrosis. Treatment with high-potency topical corticosteroids resulted in significant improvement, though scarring alopecia persisted. EPDS is often underdiagnosed due to its similarity to other conditions, but early recognition and treatment are crucial to prevent permanent alopecia. The study emphasizes the importance of gentle scalp care and protection from trauma and sun exposure.
