Dissecting cellulitis of the scalp (DCS) is a chronic inflammatory disorder leading to scarring alopecia, traditionally managed with systemic retinoids, antibiotics, and surgery, though responses vary. Recent insights into its immunological overlap with other disorders have expanded treatment options to include biologics targeting TNF-α, IL-17, and IL-23, as well as JAK inhibitors. This review highlights the promise of these biologics and small-molecule inhibitors for refractory cases, advocating a stepwise, phenotype-driven approach where systemic retinoids are first-line, and biologics are considered for severe or resistant cases. Further studies are needed to optimize treatment strategies.
