TLDR Persistent ED in Propecia users is not due to abnormal blood flow.

This study assessed erectile hemodynamics in 35 men with persistent erectile dysfunction (ED) after using 5-alpha reductase inhibitors (5-ARI). The participants were divided into two groups: 27 men who used Propecia for alopecia prevention and 8 men who used 5-ARI for benign prostate hyperplasia (BPH). The results showed that erectile hemodynamics were normal in Propecia users. However, 2 out of 8 men in the BPH group exhibited abnormal Doppler ultrasound results, indicating impaired cavernosal artery inflow. These abnormalities were potentially linked to vascular risk factors rather than 5-ARI exposure.
