Exosome-mediated interventions offer a promising new approach for treating alopecia by enhancing hair regeneration through mechanisms such as stimulating the Wnt/β-catenin pathway and promoting angiogenesis. A review of 80 studies, including 11 clinical trials with over 330 patients, demonstrated significant improvements in hair density, shaft thickness, and patient satisfaction, with effects lasting up to 1 year. These interventions, derived from various sources like mesenchymal stem cells and hair follicle cells, showed superior immunoregulatory and regenerative benefits compared to traditional treatments. However, further standardization, regulatory approval, and large-scale trials are necessary before these treatments can be widely adopted in clinical practice.
